Referral Info
How can a person receive hospice care?Anyone may refer a patient to Legacy Hospice. Referrals may come from doctors, ministers, neighbors, family members or the patient; however the patient's consent and attending physician's authorization are required for admission. Patients and their family, discharge planners, nursing homes, facility staff and physicians all can request hospice care. The patient’s primary physician is contacted to assure eligibility for services. Nursing home patients are more likely to enroll in hospice care at the end of life if their health care team is involved in the decision to refer them to the program. And when they do enroll in hospice, their families tend to be more satisfied with the care they get. Those are the conclusions of a study published recently in the Journal of the American Medical Association. Our Information and Admissions staff are highly experienced and trained to help healthcare professionals ensure that patients and their families understand their care options and connect with the right services at the right time. For more information and to discuss services or arrange for admission, you may call Legacy Hospice at (208) 895-8686.
