A request for service may be made by a telephone call from:

• A Physician
• The person desiring services
• A family member
• Any community member
• A service agency representative
• Local Hospital/Nursing Home Discharge Planners

Legacy will consult the patient's physician to authorize an individual plan of care.

Service Covered By

Medicare

If you have medicare and qualify for home care services, Legacy accepts Medicare as payment in full.

Medicaid

If you have medicaid and qualify for home care services, Legacy accepts Medicaid as payment in full.

Insurance

If you have private health insurance, Legacy will coordinate with the insurance company and yourself regarding payment.
